Providing health care services, namely medical and nursing care for persons recently discharged from a hospital or health care facility, care for persons with developmental disabilities, pediatric care and care for medically fragile children, care for persons following brain injury, care for persons following spinal cord injury, palliative care, and geriatric care; and rehabilitation services, namely physical rehabilitation therapy, occupational therapy and medical rehabilitation

social services, namely, providing personal support by providing one-to-one companionship to high risk youth to encourage positive relationships; providing personal care assistance in activities of daily living, such as bathing, grooming, personal mobility, and companionship; foster care services, namely recruitment of foster care parents, placement of foster children, supervision and assessment of foster children placements, administration of foster children placement, and supervising visits from family members; providing child and family services, namely supervising visits to observe and report on the interaction between parents/guardians and their child(ren) and to monitor and ensure safety of child(ren) with appropriate intervention when necessary.
