A holistic, highly individualized plan for ongoing treatment by a cardiologist, with the assistance of one or more other physicians or similar healthcare professionals, for patients with one or more cardiovascular conditions or at risk of one or more cardiovascular or related conditions, which ongoing treatment plan consists of a variety of components selected by the physician(s), namely, a dietary regime with education and counseling, an exercise regime with education and potentially with counseling, and possibly prescription of medications, as well as office visits at regular intervals by the patient for examination, testing, interviewing and coaching by one or more physicians and/or other health professionals over a sustained period of time in order to track the progress and effectiveness of treatment
