Providing in-home support services to elderly people, namely, geriatric care management services and personal affairs management services in the nature of coordination of necessary services and care; providing in-home support services to elderly people, namely, geriatric care management services and personal affairs management services in the nature of coordination of necessary services and care, with such services being subsidized to defray a portion of the daily costs of such services for a specified period of time; non-medical home care services for assisting with daily living activities of elderly people, such as bathing, grooming and personal mobility; non-medical home care services for elderly people to enable them to continue living at home, namely, personal care assistance for activities of daily living, such as bathing, grooming and personal mobility; non-medical home care services for elderly people in the nature of personal assistance services related to the coordination of necessary assistance with emergency medical alert systems, obtaining medication dispensing units, arranging for transportation, arranging for companion and homemaker services, and arranging for home health aide and adult day care services; non-medical home care services for elderly people in the nature of personal assistance services related to consultation and guidance concerning wellness assessments and care plans, obtaining home safety assessments and plans, obtaining assessments concerning changes in health status and functional abilities, and assisting with decisions relating to placement in assisted living facilities, skilled nursing facilities and the Program for All Inclusive Care for the Elderly
