Providing patient case management services, namely, coordinating the procurement and administration of medical-, personal-, and social-care services; providing patient advocacy services, namely, coordinating and assisting patients with filling prescriptions, completing medical forms, contacting insurance companies and obtaining authorizations; non-medical in-home personal care services for assisting with instrumental activities of daily living (iADLs) for pediatric, adult, and geriatric clients, namely, domestic tasks in the nature of housekeeping, laundry, meal preparation, clerical work, pet care assistance, transportation, and shopping and running errands; social services, namely, in-home companionship services for pediatric, adult, and geriatric clients; bereavement consulting and counseling; grief counseling; religious pastoral counseling; providing social service in the nature of companionship services and bereavement counseling as rendered by organized and coordinated volunteers and care givers for individuals and families in need; providing case management services, namely, coordinating medical and social services for pediatric, adult, and geriatric clients that connect them with the necessary community resources and supports their need to achieve a higher level of well-being
