Beacon Health Partners is an Accountable Care Organization (ACO). We are a collaboration of primary and specialty care physicians from across Long Island and Queens dedicated to achieving a system of high-quality, coordinated healthcare that supports physicians and reduces unnecessary costs while providing a multitude of benefits to patients. Beacon operates a three-pronged approach: by creating healthier physician practices and developing healthier patient practices, we ultimately aim to achieve a healthier population. Beacon flips the old model of healthcare (sick care), redirecting resources toward prevention and wellness and refocuses the emphasis onto patient-centered care. This means patients gain increased access to physicians, a better understanding of their personal health and well-being, and a reduction in unnecessary encounters with the healthcare system. Beacon's staff and infrastructure enable patients to form stronger relationships with their physicians (healthier patient practices), ensuring better communication, a reduction in gaps in care (healthier physician practices), less confusion and better patient outcomes (achieving a healthier population)
