Providing personal support services for patients and families of patients, namely, emotional and social support; Conducting day programs for adult patients with memory impairment and diagnosed dementia, namely, small-group and lunch clubs in the nature of social conversations while having a healthy lunch and doing activities; Providing personal support services for caregivers of patients with memory impairment or diagnosed dementia, namely, emotional counseling and support, in peer groups and private sessions, from inception of diagnosis to bereavement
