Home health care and evaluation services, namely, hospice care provided to patients in their residence, hospital, skilled nursing facility, or assisted living facility to maintain quality of life; Palliative care services provided in residences to patients suffering with a chronic illness, terminal illness, undergoing surgical recovery, needing medication management, needing wound care, undergoing disease or illness treatment, namely, symptom management, patient teaching, patient education, medication monitoring skilled home nursing services, speech therapy, physical therapy, occupational therapy, wound care, diabetes education and care, oncology care, cardiac rehab, nutritional counseling, massage therapy, and lending trained assistance of therapy dogs to visit the patients; Medical clinic services, namely, flu clinics, blood pressure screenings, cholesterol screening; Adult health evaluation program, namely, medical diagnostic testing, monitoring, and reporting services in order to clinically evaluate at-risk patients to detect any declines in health or exacerbation of disease, report changes in status to a patient's physician in order to prevent the patient from being hospitalized and allow out-patients to remain in their homes for as long as possible
